Efectividad y agresividad de la quimioterapia paliativa en pacientes oncológicos en etapa terminal: una revisión sistemática

RESUMEN
La quimioterapia paliativa se utiliza en pacientes con cáncer avanzado o terminal con el fin de aliviar síntomas o prolongar la supervivencia, aunque su eficacia al final de la vida sigue siendo controvertida y se asocia con efectos secundarios graves y hospitalización. Por ello, el objetivo de esta revisión sistemática fue evaluar la efectividad y seguridad de la quimioterapia paliativa en pacientes con cáncer terminal, así como analizar el impacto de la integración temprana de los cuidados paliativos en la calidad de vida y en la reducción de intervenciones innecesarias. La metodología se basó en una revisión sistemática siguiendo las directrices PRISMA 2020, realizando búsquedas en PubMed, Scopus, Embase y la Biblioteca Cochrane, e incluyendo estudios observacionales y ensayos clínicos en adultos publicados entre 2014 y 2024. Los resultados revelaron que entre el 25% y 65% de los pacientes con cáncer avanzado reciben quimioterapia en las últimas semanas de vida, y que esta terapia no mejora significativamente la calidad de vida, solo proporciona un pequeño beneficio en supervivencia y se asocia con tratamiento agresivo y hospitalización. La integración temprana de los cuidados paliativos permitió reducir la quimioterapia innecesaria, mejorar la calidad de vida y favorecer un final de la vida más digno, abordando dimensiones físicas, emocionales, sociales y espirituales. Los beneficios incluyeron la identificación temprana de necesidades, reducción de síntomas y apoyo emocional y psicosocial a pacientes y familiares, promoviendo la toma de decisiones informada y mejorando la experiencia global al final de la vida.
